Non-Identifying Background Information
As an adoptee, you can request non-identifying background information, including social and medical history, which was provided to LSS at the time of your placement.
Identifying Background Information
To get identifying information as an adoptee, you may need to secure a court order from the county where your adoption was finalized. This process involves petitioning the court to authorize LSS to release adoption records. Once a court order is granted, you must submit a written request along with any necessary fees.
Birth Parent Information Update Form
If you're a birth parent, you can keep your information up to date by completing a Birth Parent Information Update Form from LSS. This form allows you to update your contact details, which helps make a potential connection with your birth child possible. The updated information will be kept on file until requested by the adoptee. Please note that identifying information can only be shared with birth parents if the adoptee has given consent.
